Staples UEC Project Submission Form

Directions: Educators and students should complete this form and email it to staplesuec@earthforce.org. Along with the form, please attach a creative submission for your action project. This allows the judges to see the personalities the students bring to their work so please encourage them to get creative! Options for creative submissions may include, but are not limited to:

• Video diary of the project

• Prezi (www.prezi.com) or Powerpoint presentation

• Podcast or series of podcasts about the project

• Photo essay or slideshow

• Poetry or creative writing

Please submit one form for each participating group. For ease of judging, please have students fill in the form on this document and save it with the teacher/facilitator last name and city (ex. Flores_Denver.doc). Email entries to staplesuec@earthforce.org with the subject “Staples UEC entry.”
